Prospective patients require a referral from a physician (family doctor or other specialist) or nurse practitioner to be assessed in consultation by Dr Zywiel.
If possible, we request that referring providers use the standardized referral form linked below, including supporting documentation as appropriate. Completed referral forms can be faxed to Dr Zywiel’s office at 416-603-3437.
Referrals are triaged in the order that they are received.
